I believe most online sites do it the way you explained it in the first part. However, I think it's more dependent on the site/house rules than anything else.

I usually try to stick with the rules that Poker Star uses, since I'm the most familiar with them and they're probably the most respected tourney site. *Stars does go by the rule you explained in the first part.
