Your choices for custom casino chips are ASM (pokerchips.com), TR King, and Blue Chip for traditional molded clay chips and Nevada Jack and Chipco for ceramics.
As mentioned buying custom chips can be expensive ($1+ per chip). Chiptalk almost always has several group buys going on customs. The advantages of the group buy are that you usually get a pretty significant discount because of the volume of chips being bought and taking advantage of the wealth of knowledge from the contibuters there. The disadvantages are that you have to compromise somewhat on the design (and it won't be completely unique to you) and it usually takes a long time to hammer out the final designs and place the order.
The next best thing to custom designed chips are probably Paulson home chips. These can be found with just the denomination on them or as 'James Bond' chips.
Order samples and figure out what you like. Personally, I like the TR Kings the best.
